A violent parolee who was once arrested for slicing open a rival’s neck is on the loose after escaping a state parole office in Midtown Wednesday.

Travis Simon, 30, who was released three weeks ago, was reporting to his parole office when he learned he was going to be sent to a 45-day treatment program, officials said.

Simon promptly fled the office on W. 40th St. at Eighth Ave., and disappeared into a black BMW, authorities said.

According to cops, on March 31, 2010, Simon slashed a 29-year-old man in the face and neck in Hamilton Heights, severing the victim’s carotid artery, cops said. The victim survived the attack.

Simon was convicted of assault, and sentenced to seven years behind bars in 2011. He got sent back to prison last October after a July 23 gun arrest.
